This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of St.Vincent Hospitals and Health Services, Indianapolis, IN and the The Cardiovascular Research and Education Foundation of Indiana, Inc. St.Vincent Hospitals and Health Services is accredited by the Indiana State Medical Association to provide continuing medical education for physicians and takes responsibility for the content, quality, and scientific integrity of this CME activity.

 

St.Vincent Hospitals and Health Services designates this educational activity for a maximum of 5.0 AMA PRA Category 1 Credit(s)(TM). Physicians should only claim credit commensurate with the extent of their participation in the activity.

 

As a sponsor accredited by the Indiana State Medical Association, St. Vincent Hospital must insure balance, independence, objectivity, and scientific rigor in all sponsored educational programs. All faculty participating in these programs must complete a disclosure statement indicating any real or apparent conflict(s) of interest that may have a direct bearing on the subject matter of the continuing education program. The Indiana Pharmacists Alliance is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Participants will receive 5.0 contact hours for successful completion of this program. Statements of Credit will be mailed to participants following the program, upon completion of the required sign-in sheet and evaluation form, which will be collected before participants leave the meeting. Participants must attend all educational sessions (identified on the agenda as sessions that include presenter's name) to receive credit. Partial credit will not be given.
